One of the top ten questions we get asked in the fertility clinic
is, "How do I know if I am having sex at the right time?" The
truth is that timing is not as crucial as people think it is, al-
though if you don't have sex at all, obviously a pregnancy is
The key to getting a pregnancy is having live healthy sperm
ready and waiting for the egg in the correct place, the fallopian
tubes. As the egg is released from the ovary (ovulated) the
fallopian tube moves to the ovary and if all goes well, it catches
the egg and sucks it gently up inside the tube. The sperm now
have around 12 hours to find and fertilise this fragile egg be-
fore it dies. This is the woman's fertile time, i.e., the time in her
monthly cycle when she are most likely to get pregnant.
When you want to get pregnant, the best time to have sex is
actually 1-2 days before the egg is released, i.e., BEFORE ovu-
From research, we know that sperm can live for many days,
as long as they are kept warm and nourished. Once the sperm
have been ejaculated into the vagina, some of the best sperm
will swim through the womb and up into the fallopian tubes.
The sperm will usually stay alive and well in the tubes for the
next 48--72 hours and perhaps even up to 5 days. These sperm
are now ready and waiting for an egg to arrive.
If the couple are sexually active every 2--3 days, then there will
usually be sperm in the tubes whenever the egg arrives. If you
are sexually active less often, then you will need to know when
your egg is releasing each month and make sure that you are
sexually active 1--2 days before the egg releases.
How do I know when my egg is releasing?
This is the difficult part, and there is no
perfect test to answer this question.
The best way to guess when your egg
is releasing is to write down the dates
of the first day of each of your periods
over the last 12 months.
Then work out the number of days
from day one of each period to day one
of the next period starting -- this is your
"menstrual cycle length". For most
women, the time between day one of
this period and day one of their next pe-
riod is 25--35 days. If your cycle is very
regular, then you are lucky, as it is quite
easy to predict when your egg is likely
to be releasing. If your cycle varies by more than 3 or 4 days
every month, it is more difficult to guess when your fertile
What we know is that if an egg does not get fertilized by a
sperm, a period will come on 14 days later. So, every time your
period starts, you can track back and know that an egg prob-
ably released 14 days before. However, the same is not true
the other way around, meaning that when your period starts,
an egg is not necessarily going to be released 14 days later.
This is because eggs grow at different rates, and ovulate at
different times --- they can take anywhere between 11-21 days
to grow and release. The more regular your menstrual cycles
are the more you can estimate which day of your menstrual
cycle your egg is releasing (see Table below).
